New Patient Registration

According to the NHS rules and regulations, in order to register as a patient at this surgery everyone over the age of 16 needs to be able to prove who they are and where they live.

To register with the surgery you can come between 09:30 – 18:30 Monday to Friday.  A receptionist will ask you to fill in a form and offer you a registration appointment. You should bring the documents with you when registering to the appointment. The process for registration might take a few weeks so please be prepared in advance.

The following documents that can be produced should be originals and not photocopies. With at least one document from (A) and two documents from (B) are needed for registration. If you have previously registered with NHS GP in UK we must have his name and address without which we cannot register you.

(A)Identification

  • Current Passport (full)
  • Full UK Driving License (photo license)
  • Birth Certificate

(B)Proof of address

  • Utility bill –gas/electric/ water/ TV
  • Land line phone bill
  • Tenancy agreement
  • Council tax bill
  • Council rent book
  • Household or motor insurance certificate
  • HM Revenue & Customs documents
  • Working family or children or disabled persons tax credit/HMRC tax calculation

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records.
